Care Management and Service Coordination

Caregiving can become an unexpected and often unwanted responsibility—it can tax your patience and your energy. Even in the best of situations there will be challenging moments and conflict with elderly loved ones and/or other family members can make an already stressful situation even harder to cope with. The care managers at Specialty Care Management are professionals that provide the objectivity that is important when caring for a loved one.

We assist those individuals needing care by performing a complete medical, social and environmental assessment. The assessment is used to develop a plan of care that best meets the care recipient’s unique needs. Assessments begin with a phone consultation where basic information is obtained about the individual and are followed-up by an in-home meeting with the individual needing care.

Following our on-site assessment to identify care needs, Specialty Care Management can also provide but is not limited to the following care management services:

  • Arrange for in-home private pay home care
  • Manage and supervise caregivers
  • Provide ongoing support and regular communication to the family and all interested parties
  • Coordinate and attend medical appointments, including appointments for physical therapy, occupational therapy and speech therapy
  • Medication management
  • Arrange for assistive devices or home management tools to help ensure safety
  • Arrange for placement in a facility when appropriate
  • Assist with arrangements for moving from summer home to winter home
  • Coordinate bill payment and insurance filing
  • Provide regular visits and monitoring when family members are not available
  • Crisis intervention in case of an emergency-such as a hospitalization
  • Coordinate legal help for estate planning, wills, powers of attorney, health are directives and Medicaid
  • Work with trust departments and elder law attorneys to provide needed services for their customers and clients

We often do not see the gradual changes that signal that our loved one is in a crisis. Advance planning is always the best. If you answer “yes” to any of these questions, Specialty Care Management can help.
